Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our Commitment
Holding Hope Collective is required by law to maintain the privacy of your Protected Health Information (PHI), provide you with this Notice of our legal duties and privacy practices, and follow the terms of the Notice currently in effect.
How We May Use and Disclose PHI
- Treatment: We use PHI to provide therapy, psychiatry, case management, and to coordinate care with other providers involved in your treatment, including your prior treatment team when you authorize the warm handoff.
- Payment: We use PHI to verify insurance benefits, prepare superbills for out-of-network reimbursement, and bill for services.
- Health Care Operations: Quality improvement, clinical supervision, training, and administrative functions.
- As Required by Law: Mandatory reporting (e.g., suspected abuse of a child, elder, or dependent adult), valid court orders, and public health activities.
- To Avert a Serious Threat: When necessary to prevent a serious and imminent threat to the health or safety of you or others.
Uses Requiring Your Authorization
Most uses and disclosures of psychotherapy notes, marketing communications, and any sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
Your Rights
- Inspect and obtain a copy of your records.
- Request an amendment to your record.
- Request restrictions on certain uses or disclosures.
- Request confidential communications by alternative means or location.
- Receive an accounting of certain disclosures.
- Receive a paper copy of this Notice on request.
- Be notified in the event of a breach of unsecured PHI.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at info@holdinghopecollective.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to This Notice
We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have about you, as well as any information we receive in the future. The current Notice will be posted on our website and available at our office.
